Coverage Type: Scheduled Clinical Hours Only
Schedule: Full time need Monday-Friday. We can consider one full time provider or we would look at 2 providers- 2
weeks on, two weeks off. The provider will work 2 days per week in the outpatient surgery center next door and 3 days per week in the clinic setting
Work Details:
· Support staff: Yes
· Patients per shift: 12-15
· Providers Requested: 2
· EMR: Epic
· Skills required: All epidurals, facet injections, ablations, and other procedures are performed at the surgery center.
Practice Setting:
• Outpatient
Requirements:
· Board Certification: Yes
· License required: Active CA License Only
· Certifications required: active fluro, ACLS
· case logs from the program for the previous 24 months documenting the performance
· at least 100 cases representative of the scope and complexity of the privileges requested
Credentialing Timeframe: 60-90 Days
