Job description

• Provider will be needed 10 shifts per month
• Must be WY licensed or be able to show license is in the process.
• WY DEA must have or easily transferrable one.
• Must be Board Certified
• 24 hr coverage will be needed for all patients
• Patient volume: 4-5
• Support Staff: Nursing, R.T
