Hours: Monday – Friday, 8:00 am – 12:00 Noon

Facility Type: Employee Health and Wellness Center

Practice Setting: out-patient, primary care clinic

Board Certification(s): Board Certified or Board Eligible in Family Medicine or Internal Medicine

Age Group: Adults

Credentialing Timeframe: 2 weeks

Must have a Louisiana License
