Types of Cases: Diverse mix of cases including Ortho, General, GYN, Urology, Neuro, Spine, Plastics, Eyes, Endo, EP, IR, OB, optional Cardiac (Cardiac stipend) and Neuro MRI team, bariatric No Peds/Trauma/Transplants

Required Procedures: one Required- spinal and epidural preferred but not required.

Average Time to Credential: 90-120 days

EMR: Cerner

Life Support Requirements: ACLS, BLS

Shift Days: Mon-Fri

Minimum Number of shifts per month: prefer full time coverage; minimum 15 shifts per month.

Current Available Shifts: Various shifts available including 7a-5p, 7a-8p, 10a-11p, 1p-11p, mid-shifts, open to creative shifts Flexible scheduling – 8s, 10s, 12s, 13s, 14s, 15s, 16s

Case volume/Approximate annual cases: Main OR: 17,000, OB deliveries: 2,200/ C-sections 745, Cardiac OR: 1,500 21 Main ORs, 5 Cardiac ORs, 5 Endo suites, 3 OB ORs, 3 Electrophysiology suites, 3 IR suites.



SUBMISSION REQUIREMENTS
We need providers that can start in September.
Must have active FL license
Must not have any prior malpractice history or background issues
Must have Full-time Open Availability
Availability
1st possible date they would be available?
How many shifts would they be able for per month?
What month is their schedule completely open?
Any shift preferences (Day, Night, Blocks…etc)?
Any dates in the next 3-6 months they can’t work?
Any Holidays in the next 3-6 months they would be open to work?
