Schedule: 6:30a-2:30p
Work Details:
· WILL ACCEPT 1 WEEK PER MONTH IF NEEDED
· Weeks needed: July 24-28, Aug 21-25, Sep 5-8 , Sep 18-22, Sep 25-29, Oct 2-6 , Oct 9-13 , Oct 23-27, Oct 30-Nov 3, Nov 6-10, Nov 13-17, Nov 20-23, Nov 27-Dec 1, Dec 11-15, Dec 18-22 , Dec 26-29
All Adult Patients
No Heads or Hearts
Will License if needed!
Credentialing Timeframe: 30-45 days
Rate: $200/hr - T&L covered + Med mal insur
