Job description

o Location: Louisville, KY
o Assignment Duration: Ongoing coverage needed.
o Schedule: Monday-Friday. 8 or 10 hr shifts
o Weeknight and Weekend Pager Call Available
o 18 ORs at hospital
o 10 ORs at PAV
o Care Team Model – Medical Supervision 1:3-4.
o Case Mix: General, Major Spine, ENT, Ortho, Vascular, Cardiac, OB, Trauma, Peds
o Credentialing Timeframe: Approx. 60-90 days to process a completed credentialing application.
