Empower Nation is seeking a Licensed Clinical Worker with extensive background in behavioral health management to perform weekly engagement with members via tele-health to support and empower their progress towards their 5 -year success plan

As a Licensed Counselor, you will play a critical role in identifying and establishing resolutions of problems that may adversely affect achievement. This position will conduct weekly contact sessions with caseload members to provide understandable support and plans to members and to listen carefully to understand their needs. The licensed counselor will work diligently with the members to expand on their strengths and empower their capacity to succeed.

Key Responsibilities:

Evaluate members (existing, new and prospective) based on their needs, limitations and desires

Address patient concerns and goals while maintaining weekly communication with the member as well as a case manager

Build rapport with members and their families, if necessary

Coordinate care with Junior Success Agents to address needs and to promote the over-all well-being of member

Complete psychosocial intake assessments and ongoing clinical assessments

Develop collaborative treatment plans with members

Provide management of assigned cases, including monitoring, and reviewing Individual Treatment Plans, Safety Plans, and Transition/Discharge Plans.
