1-Job Description: Phoenix, AZ
· 75 ED Beds
· 85,000 annual volume, 83% adult, 17% Peds
· 10-12-Hour Shifts
· Level 4 Trauma
· EMS arrivals: average 35 a day
· Advanced Practice Provider coverage
· Scribe coverage
· All specialties available except: Pediatrics, Neurosurgery, ENT, and OMFS.
· 3rd Busiest ED in AZ
· MUST be Current for Board Eligible ONLY
· MUST be EM residency trained or ABEM/AOBEM certified/eligible
· All Licenses, Boards and Certs with expiration dates must be listed on CV
· Anyone with malpractice must provide a NPDB

STATE LICENSE REQUIREMENTS
· Arizona
2- Job Description: Surprise, AZ
· 80 to 100 patients per day
· 13 beds
· Resuscitation room
· 10 partitioned off hall beds
· 24 hours of APP coverage
· 24 hours of Physician coverage
· Scribe coverage for each shift
· CT scanner with 24-hour radiology coverage
· Call in techs for sonography
· Ultrasound availability 24 hours/day
· On site point of care testing for lab evaluation
· EPIC EMR
· Accepting BC/BE in EM only
