• 20k Annual volume
• ATLS;ACLS;PALS
• BC/BE (ABMS) EM pref'd, FM, or IM Must be BC w/in 5 years post residency. ACLS, ATLS & PALS required. Active FL Medical License, DEA (with FL address), case logs past 2 years, CMEs past 2 years, copies of all education & training diplomas, SS Card, PPD, Flu Vaccine, Driver's license, color photo
