Home Health Family Nurse Practitioner Needed in Oregon - PT or FT!

Nurse Practitioner – Home Health Assessments
Lane, Multnomah or Washington County, OR

Start Date: March 2026
Contract Length: ongoing contract
Schedule

Monday–Friday
8:00 a.m.–5:00 p.m. with a 1‑hour lunch
Full‑time (40 hours/week) or part‑time (20 hours/week) options
Standard daytime schedule; no call
Schedules released 2–3 weeks in advance

Practice Details

Home‑based patient assessments within a regional float model
Assessment‑only role; no treatment or prescribing
Conduct 5–6 in‑home health assessments per day
Perform annual risk assessments and chronic condition reviews
Escalate urgent findings to the patient’s PCP
A1C testing only when applicable
Orientation provided (1–3 hours); additional modules to be completed during downtime
Provider must be enrolled/credentialed in Medicare

Clinical Scope

Complete structured in‑home evaluations
Review chronic conditions, capture diagnoses, and update medical histories
Document all encounters per program guidelines
Maintain timely communication regarding escalations
Utilize provided tools and resources for documentation and testing

Requirements

Nurse Practitioner – Family Practice
Active state license required at submission
Board Certified Family Nurse Practitioner
Must be credentialed/enrolled with Medicare

Credentialing

Credentialing documents must include active state license and all required certifications
Education modules must be completed within 30 days of start
