Job description

"Board Certified FP/IM (will not take Board Eligible) Physician
Dates coverage is needed: ASAP – ongoing (min 3 month commitment rqr’d)
Schedule/Shifts Times: Monday – Friday 8a – 5p (1 hr lunch)
Call Requirements: None
% inpatient / % outpatient: Outpatient only
Patient Volume: 12- 15 patients per day average
Patient Ages: adults only
Types of Cases: standard outpatient care, chronic illness treatment
Support Staff: yes (several Physicians, mid-levels, RN’s and specialty Physician support)
Procedures Required: None
Requirements: BLS, ACLS (through American Heart), DEA (fee paid, schedules 2-5)
Reason for Need: staff shortage
Credentialing Timeframe: 4-6 weeks on average
