Momentum Healthcare Staffing is seeking a Primary Care Physician located in Dayton and Cleveland, TN for a permanent position.

Requirements:
Specialty: Primary Care
License: TN
Certifications: Board Certified in Family or Internal Medicine
Education: MD/DO
Experience: Experience in a Community Health Center a plus
Average Caseload: 20-25
Patient Population: all ranges, no infants

Schedule:
Hours: 8am to 5pm M-F
Must be full time

Salary/Benefits:
Pay range: $195k
Health Insurance premiums paid for the Staff Member $1008.02 per month
Life Insurance paid for the Staff Member $250,000.00 (must pass screening)
401K 4% paid in addition to salary
25 days of PTO (paid time off)
10 Federal Holidays Observed
$2,000 Professional Annual
Birthday Leave
Malpractice insurance to include tail coverage through FTCA
