Schedule:
• Flexible scheduling options available: Full-Time, Part-Time, or PRN
• Operating hours: Monday–Sunday, 7a-7p
• Providers may choose preferred shifts, (ie 8 hr, 10 hr, 12 hr shifts, etc)
• Prefer those who can work at least 60 hours per month.

Work Details:
• Average visits per day: approximately 1 patient per hour, 30-45 minute assessments timeslots. They will factor drive time in and traffic for appointments, especially in the afternoon
• Department Description: Conduct pre scheduled In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. These visits include a medication and medical history review and a physical evaluation performed in a health plan member’s home.
• They will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.
• Some at home testing may be required (ie testing A1C levels, cholesterol, etc)
• Providers must be comfortable entering homes where:
o Smoking may occur
o Pets may be present
• No prescribing or diagnosing - DEA is not required
