✅ Key Responsibilities
✔️ Administer medications and prescribed treatments
✔️ Monitor and document patient vitals and health status
✔️ Support care plans in coordination with healthcare team
✔️ Assist residents with ADLs and mobility needs
✔️ Maintain accurate medical records and documentation
✔️ Follow infection control and facility safety protocols
✔️ Respond promptly to patient emergencies

✅ Qualifications
✔️ Completion of accredited LPN program
✔️ Active NY State LPN license required
✔️ Minimum 1 year long-term care or hospital experience
✔️ Must provide proof of Hepatitis B immunity
✔️ Strong teamwork, communication, and adaptability skills
✔️ Comfortable working rotating weekends

• 1 Week of Paid Vacation based on accruals after 3 months of employment
