Home Health Adult NP or FNP Needed in NC (QUICK START)

Location: Burlington & Greensboro, NC

Start: August

Contract Length: 12 weeks with the potential for longer

Shift Details: Mon-Fri (40hrs/ week)
Schedule: M-F 8a-5p (40hr/ week)

Work Details:
· Amount of coverage needed per month: 1 FTE
· Patients per day: Combo of telehealth and home visits: 6pt/day for home visits (Apts approx. 45 min); 8pts per day for telehealth (All equipment provided)
o Conducts physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers. Focuses on health promotion, disease prevention, and patient education to ensure continuous and holistic care for home-bound patients.
· Working out of (Greensboro and Burlington): Traveling to make home visits in the surrounding 5 counties.
o They will be working in a regionalized area per day but the areas can change from day to day.
o Scheduler confirms the appointments the day before
· Patient population/age: Geriatric
o Will be seeing a combo of New and Existing patients
· Department description: Home Health/ TeleHealth
· EMR: Athena (Experience preferred or must do modules before arrival)

Practice Setting:
• Home Health with Telemedicine

Requirements:
· Board Certification: FNP or Adult NP
· License required: NC license required
· DEA: NC DEA Required
· Certifications preferred:
· Certifications required: American Heart Association or American Red Cross Basic Life Support (BLS) is required.
· MUST HAVE: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
· Minimum experience required: 2-3 years experience in an acute care, home health, long-term care, or outpatient/office setting required. Palliative care experience preferred.

Credentialing Timeframe: 1-2 weeks
