Location: Greensboro & Burlington, NC (surrounding 5 counties)
Start Date: ASAP
Contract Length: 12 weeks (potential to extend)
Schedule: Monday–Friday, 8a–5p (40 hours/week)
Preference: Drive-in candidates only

Join a compassionate and dynamic team providing comprehensive home-based and telehealth care to adult and geriatric patients. This role blends clinical independence with multidisciplinary support, allowing you to make a meaningful impact in patients’ homes and communities.

Position Highlights:
• Scheduler confirms appointments daily
• Work regionally across designated counties (travel varies day to day)
• EMR: Athena (experience preferred; training available)
• Credentialing timeframe: 1–2 weeks

Responsibilities:
• Conduct physical exams, diagnose and manage chronic and acute conditions
• Coordinate care with other providers and engage in advanced care planning
• Emphasize preventive care, patient education, and holistic wellness
• Maintain thorough documentation in alignment with CMO standards

Requirements:
• Active North Carolina NP and DEA licenses
• Valid driver’s license, auto insurance, and personal transportation
• Minimum 2–3 years of experience in acute care, home health, long-term care, or outpatient settings (palliative care preferred)
• Must be comfortable managing complex patients (often with 10+ comorbidities)

• Mix of home visits (avg. 6/day, ~45 min each) and telehealth visits (avg. 8/day)
• Credentialing timeframe: 1–2 weeks
• Full-time locums assignment
