TinkBird Healthcare Staffing is seeking an experienced Nurse Practitioner for a permanent opportunity with a PACE program serving geriatric patients in Greensboro, North Carolina. This position offers the opportunity to work collaboratively within an interdisciplinary care team focused on providing comprehensive healthcare services to chronically ill and medically complex elderly participants.

Clinical Responsibilities
Perform comprehensive health histories and physical examinations
Assess and manage healthcare needs of chronically ill geriatric patients
Order, interpret, and follow up on laboratory and diagnostic testing
Initiate emergency measures and coordinate appropriate referrals when indicated
Prescribe and manage medications within physician-approved protocols
Coordinate daily patient care in collaboration with supervising physicians
Request specialty consultations and coordinate ongoing care management
Provide patient and family education
Serve as a liaison between physicians, nursing staff, patients, and families
Participate in interdisciplinary care team meetings and patient care planning
Communicate changes in patient condition to care team members
Maintain accurate and timely clinical documentation

Requirements
Bachelor’s degree in Nursing from an accredited program
Graduate of an accredited Nurse Practitioner program
Active North Carolina RN license required
Nurse Practitioner certification required
Minimum 3–5 years of nursing experience
Minimum 1–3 years of Nurse Practitioner experience preferred
At least 1 year of experience working with frail or elderly patients required
Strong analytical, teaching, and problem-solving skills
Ability to work collaboratively in a team-based environment
Must meet CMS competency requirements prior to independent practice
Current immunizations and medical clearance required
