Join a forward-thinking team as a Registered Nurse for the PACE (Program of All-inclusive Care for the Elderly) program in Downtown Los Angeles. In this essential role, you’ll deliver high-quality, integrated nursing care to a diverse panel of frail older adults and adults with complex needs across multiple settings—including clinic, adult day center, home visits, virtual care, and skilled nursing facilities. As a key member of the Interdisciplinary Team (IDT), you’ll conduct comprehensive assessments, contribute to care planning, and coordinate transitions of care. You’ll practice at the top of your license, triaging, initiating therapies, and educating participants and caregivers.
