This Program of All-inclusive Care for the Elderly (PACE) assists older adults in living safely at home while providing comprehensive medical, health and social services. The facility serves eligible participants who are 55 years old or older, live in the designated service area, and meet state requirements for nursing home care. Services include adult day health programs, rehabilitation therapy, nutritious meals and social engagement delivered both at the center and in participants' homes.

About the clinician's workday

The physician associate will provide comprehensive geriatric medicine services Monday through Friday from 8:00am to 5:00pm in a PACE clinic setting. Responsibilities include delivering primary medical care to eligible participants aged 55 and older who meet state requirements for nursing home care. The position involves managing both acute and chronic conditions while coordinating comprehensive medical, health and social services to support patients living safely in the community.
