Job description

Work within a collaborative interdisciplinary team (IDT) approach to guide participant care. The program provides seniors with the opportunity to continue living in their communities. The innovative and comprehensive range of medical services is what ignites the passion to treat the whole person and not the symptoms!

The program serves as the patients’ health insurer as well as directly provides primary care and social services to keep them living safely in their communities.

This is an ideal job for a clinician, as it allows you to spend ample time engaging with and truly caring for patients without the mis-aligned incentives of fee-for-service.

The Primary Care Physician performs in-person comprehensive assessments and coordinates with the IDT to develop a care plan for each participant. At the direction of the Medical Director, the Primary Care Physician oversees all care of participants in nursing homes and hospitals including admits and discharges for acute, short-term, and long-term placement and the use of medical specialists to ensure effective care coordination. The Primary Care Physician also provides direction and support to Nurse Practitioners and other members of the IDT.

MD/DO will have Board Cert – Internal Medicine, Family Practice, Geriatrics and/or with Geriatrics experience.
Current knowledge of chronic care/geriatric issues and best practices.
