Nurse Practitioner - Hospice - Pallative Care needed in Ohio
Job description
Provide outpatient palliative care services including home visits, extended care facility consultations, telehealth, and clinic-based care.
Manage 6–8 patients per day based on location across a 9-county area in NW Ohio.
Participate in delicate, goals-of-care discussions with patients and families facing serious or terminal illnesses.
Assess, plan, implement, and evaluate individualized care needs in alignment with palliative care principles.
Collaborate with a supportive interdisciplinary team including other NPs, physicians, and administrative staff.
Participate in care transitions, including determining when patients would benefit from continued palliative care or hospice services.
Work closely with a hospitalist team responsible for all patient admissions.
Document patient encounters thoroughly and maintain excellent communication with patients, families, and care teams.

Compensation and Benefits:

#Board Certified NP - REQUIRED
#OH license (or willing to obtain) - REQUIRED
#Experience as advanced provider in an acute care setting or with palliative care and hospice (NO EXCEPTIONS) - REQUIRED
