JOB SUMMARY:
The Director of Patient Care Services (DPCS) provides oversight and support of all patient care and associated personnel for assigned program(s). This oversight includes patient and personnel assignments, coordination of care, coordination of referrals, assessment of patient needs, and ensuring the plan of care is developed, implemented and updated. The DPCS is responsible for the services provided by the home health and hospice programs and strives to provide the highest quality of care through all service lines.

JOB DUTIES:
The DPCS is responsible for their assigned program(s) lines of service and overall results compared to goals. They will effectively ensure the facilitation of required Interdisciplinary Group Meetings, Case Conferencing, and Coordination of Care. They will investigate and report any problem relating to patient care or conditions which might harm the patient and/or employee well-being following appropriate processes. Consults with the COO/Administrator to determine appropriate staffing which will accomplish stated objectives and promote maximum level of utilization of personnel. Is responsible for recruiting, hiring, evaluating and terminating personnel in collaboration with Human Resources. Directs the planning and completion of orientation for new employees which may include conducting selected orientation classes. Participates in the Program’s ongoing Quality Assessment and Performance Improvement (QAPI) program through collaboration with the Compliance Officer. Oversees appropriate data collection, aggregation and analysis, to develop appropriate goals for the program’s QAPI plan and reports on outcomes. The DPCS is a key member of the leadership team and works closely with other leaders to coordinate internal services with services of other community agencies, to grow our presence in our service area and to ensure a positive and healthy working environment. Working in collaboration with the Business Development team the DPCS will identify opportunities to develop new lines of service and referral sources. This may include support in the planning of outreach campaigns, identifying barriers to access, and educating staff and providers about the clinical capabilities of assigned programs. The DPCS will primarily be focused on internal marketing with respect to the excellence of the clinical program, great customer service, and expansion of clinical programs. They will also have responsibility for communicating these results to the community at large, keeping them informed of our clinical excellence, customer services, and development/implementation of expanded services. The DPCS will provide targeted outreach education for existing providers and support the establishment of new provider relationships. The DPCS outreach goals shall include continued active relationships with providers and community activities that provide opportunities to grow census through identification of provider’s/patient’s goals and needs and ensuring our services are clearly understood.

Education/Experience/Skills/Qualifications:
Possession of a current RN license to practice in the state of California and current CPR certification. Three to five years’ experience as a Home Health Supervisor or Hospice Manager. Ability to exercise independent judgment and critical thinking to supervise and coordinate clinical professionals and agency support staff. Ability to learn to effectively operate computer hardware and software programs.
