Job Summary:
Assumes the authority, responsibility and accountability for directing the home care and/or
hospice programs under the supervision of the Clinical Manager and as back up to the Clinical
Manager in their absence. Actively participates in the management of clinical aspects and
quality assurance of the home care and hospice programs, and the selection, supervision and
optimal utilization of qualified personnel. Implements policies and procedures. Facilitates clinical
management of patients. Coordinates and attends patient care planning conferences.
Participates in patient care assessment, planning, intervention and evaluation. Participates in
licensing surveys and reviews. Serves on or leads various committees. Promotes favorable
public relations.

Responsibilities
Provides leadership to staff to accomplish agency vision and mission. Participates in the review
of documentation of patient care for completeness and appropriateness; participates in and
assure staff participation in weekly case conference meetings to assure appropriate clinical,
financial, and utilization management of the patient care; ensures patient’s rights are well
established and maintained at all times; rotates position as on-call nursing supervisor when
scheduled; performs duties of those supervised as necessary (skills as applicable to licensure);
aides as a clinical resource nurse to the home care and/or hospice nursing staff; makes field
supervision visits with staff as needed; assists with yearly competencies; discovers training
needs and provides coaching.

Assists in the supervision of home care and/or hospice staff and other contractual personnel as
assigned; assists the Clinical Manager in scheduling and overseeing staff to ensure optimal
productivity, effective utilization and provision of services, and organizational standards and
requirements; delegates authority, responsibilities, tasks, and accountability to other responsible
personnel as appropriate; communicates company goals, policies, procedures, and changes to
staff; assures performance evaluations are completed timely; ensures staff maintain compliance
with professional license, CPR, driver license, TB screening, health assessments & mandatory in
services and training programs with support from Human Resources; assists with the orientation
and supervision of newly hired staff; participates in review, approval, and/or follow through with
staff payroll, patient medical supply orders, clinician mileage, incident reports, staff PTO
requests, and budget reports.

Education/Experience/Skills/Qualifications
Possession of a Registered Nurse License (RN) to practice healthcare in the State of California
and current CPR certification, with at least 2 years of recent experience in home care or hospice,
preferably with 1 year of supervisory experience or equivalent training in a home health or
hospice agency. Excellent communication, leadership, organizational, decision-making, and
time-management skills. Working knowledge of: Title XXII, HIM-11, Medi-Cal requirements,
agency policies and procedures.
