Key Responsibilities Include:

Performing comprehensive assessments and creating individualized care plans.

Teaching self-care techniques and offering health counseling.

Acting as a Case Manager when assigned, ensuring continuity and quality of care.

Participating in team meetings and supporting hospice goals.

Tools/Environment: In-home visits, electronic documentation systems, collaboration with interdisciplinary hospice team.

Schedule: Day shifts with flexibility depending on patient needs.

Travel Required: Yes – must have a reliable, insured vehicle and current driver’s license.
