✔️ Provide primary and acute care to nursing home patients
✔️ Diagnose, treat, and manage acute and chronic medical conditions
✔️ Develop, implement, and review individualized care plans
✔️ Collaborate with nurses, specialists, social workers, and families
✔️ Maintain accurate and timely medical documentation
✔️ Participate in care conferences and family meetings

✅ Qualifications
✔️ Education: MD or DO from an accredited medical school
✔️ Licensure: Active New York State Medical License (Required)
✔️ Certification: Board Certified or Board Eligible in Internal Medicine
