Car, fuel, mileage, hotel is a $150 cap (including taxes)rental must be refueled before being returned
McLaren does NOT reimburse Med Staff Fees
No lodging booked through 3rd party websites, ie expedia, kayak, etc
McLaren is tax exempt and will not reimburse taxes on lodging
Malpractice rates MUST be included in the rate
Orientation rate is 1/2 the hourly rate

Site uses reverse billing - provider must clock in/out using DocTime - no timesheets will be approved

Submission Requirements

ALL MUST BE NOTATED AT TIME OF NAME CLEAR
Board Certified OBGYN (No BE) - [REQUIRED]
MI License & CSR at Submission - [REQUIRED] at time of clear, no exceptions - not accepting IMLC
BLS - Required

Provider must be local - [REQUIRED], no exceptions

Must have clean background for quick credentialing- [REQUIRED] no exceptions
Please submit rates at time of name clear - [REQUIRED] will not send to client without rates

Please provide all availability if not available for listed dates about - [REQUIRED]
DEA - Verbal Confirmation [REQUIRED] at Name Clear
Expenses covered by client will not exceed the limitations listed in the Travel & Expense policy between McLaren Health Care & AHSA.
