Job description

Role of Occupational Therapist:

Conduct comprehensive fall-prevention assessments and written recommendations for each home

Provide at least two in-person visits per client (initial assessment + follow-up)

Evaluate outcomes and monitor effectiveness of home repairs or modifications

Collaborate with program staff to ensure timely completion of fall-prevention interventions


Candidate Requirements:

Licensed Occupational Therapist (OT)

Board certification required

Comfortable working independently in community/home settings

Strong documentation and assessment skills
