JOB DESCRIPTION:
Mon-Friday 8-5
50/50 kids/adults
The provider must be willing to work 3 months minimum with the option to extend.
No exceptions.
Chart closure within 72 hours
A decision is typically made within 7-14 days
Active DEA needed before start
COVID-19 initial vaccine required; boosters are optional
HCO & Vendor agree to a 30-day notice for ending all assignments.

Coverage Type:
• No Call
• Outpatient
• Patient per shift: 16

REQUIRED CERTIFICATE:
• Board Certificate
• BLS
