Job description

- Dates of Coverage: ASAP – 1 Year with options to extend
- Practice Description:
- Setting: Clinic
- Daily Responsibilities: Clinic Setting with clinic patients, new consults, follow ups, minor procedures in
clinic
- Shifts/Schedule Expectations: Monday – Friday – 8:00am – 5:00pm – No Weekends, No Holidays
- Call Schedule/Responsibilities: No Call but open to discussing if provider is wanting
- PPD/Census: Estimated 18-20 PPD Will work in basket work as well (Emails, Follow-ups, etc.).
- EMR: EPIC (Health Connect – Kaiser Permanente’s version of EPIC)
- Certifications required/accepted: Board Certified or truly Board Eligible
- Credentialing Time frame: Estimated 2 Months upon application submitted
- Will a licensing candidate be considered : Yes
