IMLC or WI license required
Asap start temp priv
$250 Hr
Outpatient
2 wks on 2 wk off
Days week and time is flexible
EMR-Epic
Avg 15 ppd

2 weeks on 2 week off, flexible for 4 to 5 days per week, 7a-3p (flexible on time). 
Office visits, follow-up visits, procedures
Procedures include spinal cord stimulation, radio-frequency ablation, injections  

Act 10 is acceptable.
