Our Mental Health Center is seeking a full-time OneCareKansas Care Manager. This position will work with assigned OneCare Kansas (OCK) clients in meeting their goals as set forth in the client’s Health Action Plan through the provision of the six core OCK services: Comprehensive Care Management, Care Coordination, Health Promotion, Comprehensive Transitional Care, Member and Family Support and Referral to Community Supports and Services. This position will also work closely in coordination with all health providers serving the client.

Qualifications:
Applicants must have a High School diploma.
Certification in the State of Kansas to provide this service is required.
A bachelor's degree from an accredited university in the various behavioral health or medical fields such as psychology, social work, counseling, nursing, rehabilitation, or closely related degree is preferred.
Experience in lieu of or in addition to education may be considered.
Previous experience in a community mental health center working with mentally ill or substance abusing consumers is preferred.
Experience in the person helping professions and occupations is desired and helpful.
