Schedule Hours for Clinic /Hospital- 8a-5p
Schedule Days Clinic /Hospital- M-F
Number of patients seen per day in clinic- 20
Call Coverage:
• Call Night /Weekend- Yes
• What are the days for call- Sun-Sat
• What Time does the call start- 5p
• What Time does call end- 8a
Coverage Type:
Age of patients- 18+
Inpatient/ Outpatient - Both
Any procedures required- Radiation Oncology Procedures
Type of Charting /EMR- Meditech
Board Certified
Is Fellowship required- Yes
How Long Does Credentialing Take- 3 months
Are Temp Privileges Granted- Yes
Travel and Lodging (will you pay)- Yes

REQUIRED CERTIFICATION:
• ABLS
• ACLS
• ATLS
• BLS
• PALS
