New opening for Urology locum in Eureka, CA-November 23rd start-$410 Hr-Ca license accepted-Inpatient and Outpatient

LOCUM- UROLOGY

ACTIVE FLURO IS REQUIRED, we will verify online prior to submitting bids to the site. Please attach a copy if necessary. 

Please ensure the bid reflects: 
Guaranteed Hours: 8 
OT applies after 9 hours per day. 

OT must be approved by clinical operations. 

We will need coverage in the clinic and help with call coverage. Looking for one provider to help cover here as it will only be a week or two a month. 
      -M-Thurs (8-5 days/times may vary)
      - Friday (730-1700 days/times may vary)
Call will be 7-10 days per month including weekend, dates will vary. 

Provider will have an hour unpaid lunch during the 8am-5pm. 

If they are on taking call on a week they are in clinic, the shifts will be set up as regular hours during the day (M-F) and then night call each weekday night. Then 24hr call on the weekends. 

Dates of coverage needed:

Dependent on Privileging!! 
November 24-30 (Thanksgiving)
December 22-January 4th (Christmas and New Years)

2/9-2/16
3/23-3/30
4/21-4/29
5/26-6/2
6/22-6/29


We can only look at temps if the file is clean! Please note that the privileging timeline will not begin until the RFA and application are turned back in. Typical turn around for RFA is 48-72 hours. Application should be submitted within 5 days.

o    All specialties require 2 years of current Hospital clinical activity in the specialty.
o    Gaps greater than 3 months must be explained.
o    Include current number of case logs upon presentation for surgical specialties.  

Active CA license is required. Provider must be BC or truly BE.
